ABSTRACT

Ferroptosis is a novel form of cell death that has been discovered in recent years and differs markedly from previously known cell death. The mechanism of ferroptosis is the inactivation of glutathione peroxidase(GPX)and the accumulation of lethal intracellular lipid peroxides that occur on the basis of cellular iron overload. Changes such as cell membrane rupture, mitochondrial crest reduction, and outer mitochondrial membrane shrinkage rupture can be observed under electron microscopy. Current studies have found that many diseases in ophthalmology involve ferroptosis-related processes such as iron overload, the imbalance of redox homeostasis, the inactivation of GPX, and accumulation of lethal levels of lipid hydroperoxides, which identified the important role of ferroptosis in ocular disease. This review focuses on the mechanism of ferroptosis and its role in corneal injury, cataract, glaucoma, age-related macular degeneration and diabetic retinopathy, which helps to sort out the pathological mechanisms of common ocular diseases and provide new ideas for the prevention and treatment of ocular diseases.

ABSTRACT

ABSTRACT Purpose: To evaluate the epidemiological and clinical profiles of corneal transplants performed in a reference eye center in Recife, state of Pernambuco, Northeastern Brazil. Methods: This cross-sectional study collected epidemiological and clinical data from the medical records of patients who underwent keratoplasty at the Altino Ventura Foundation between January and December 2017. Results: A total of 356 procedures were performed in 327 patients, of whom 165 (50.5%) were female. The mean age at surgery was 50.9 ± 22.6 years (range, 10-89 years). Most patients (n=152 [46.5%]) were from the capital and metropolitan areas. The mean waiting time for keratoplasty was 52.4 ± 58.9 days (range, 0-460 days). The main indications for keratoplasty were infectious keratitis (n=88 [24.7%]), keratoconus (n=80 [22.5%]), and previous transplant failure (n=75 [21.1%]). Penetrating keratoplasty was the most common surgical technique performed (n=213 [59.9%]) and more frequently performed in men (n=132 [76.7%]), whereas posterior lamellar transplant (n=143 [41.1%]) was more frequently performed in women (p<0.001). Conclusion: Infectious keratitis was the main indication for keratoplasty, which was similarly performed in economically active adults of both sexes. Penetrating keratoplasty was more frequently performed in men and lamellar transplants in women.


RESUMO Objetivo: Avaliar o perfil clínico e epidemiológico dos transplantes de córnea realizados em um centro de referência oftalmológica de Recife no estado de Pernambuco, localizado no nordeste do Brasil. Métodos: Esse estudo transversal coletou através de prontuários médicos dados clínicos e epidemiológicos de pacientes submetidos a ceratoplastia na Fundação Altino Ventura, de janeiro a dezembro de 2017. Resultados: Um total de 356 procedimentos foram realizados em 327 pacientes dos quais 165 (50.5%) eram mulheres. A média de idade na cirurgia foi de 50.9 ± 22.6 anos (variação, 10 - 89 anos). A maioria dos pacientes (n=152 [46.5%]) era da capital e região metropolitana. A média de tempo de espera na fila para o transplante de córnea foi de 52.4 ± 58.9 dias (variação, 0 - 460 dias). As principais indicações de transplante foram ceratite infecciosa (n=88 [24.7%]), ceratocone (n=80 [22.5%]) e falência de transplante prévio (n=75 [21.1%]). Transplante penetrante foi a técnica mais realizada (n=213 [59.9%]) e foi mais comum em homens (n=132 [76.7%]), enquanto os transplantes lamelares posteriores (n=143 [41.1%]) foram mais realizados nas mulheres (p<0.001). Conclusão: Ceratites infecciosas foram a causa mais comum de transplante, com prevalência similar em adultos economicamente ativos de ambos os sexos. Transplante penetrantes foram os prevalentes em homens e os transplantes lamelares em mulheres.

ABSTRACT

Objective:To explore the promoting effects of slit guidance ligand 2 (Slit2) on the repair of corneal epithelium and nerve damage in diabetic mice and possible molecular mechanism.Methods:Sixty SPF C57BL/6 mice aged 5-6 weeks were divided into normal control group, diabetes model group and Slit2 injection group according to the random number table method, 20 for each group.Diabectic model was prepared by intraperitoneal injection of streptozotocin in the diabetes model group and Slit2 injection group.A mouse corneal epithelial injury repair model was established using electric epithelial scraper, and Slit2 recombinant protein was subconjunctivally injected immediately following modeling in the Slit2 injection group.The equal volume of phosphate buffer saline (PBS) was used in a same way in the diabetes model group.No intervention was performed in the normal control group.Corneal epithelial healing were examined at 24, 48 and 72 hours after corneal epithelial defect by corneal fluorescin staining.Real-time fluorescent quantitative PCR was used to detect the expression of Slit2 and its related receptors in the corneal epithelium of normal and diabetic model mice.Fluorescence staining of corneal wholemount with β-tubulin Ⅲ was used to observe the changes in corneal nerve morphology.Immunofluorescence staining was performed to detect the expression and distribution of Slit2 in mouse corneal epithelium in normal control group and diabetes model group, as well as the expression and distribution of Slit2, epidermal growth factor receptor (EGFR), extracellular-signal-regulated kinase (ERK), threonine protein kinase (AKT), β-catenin and Ki67 in the healing corneal epithelium of mice after corneal epithelium damage in different groups.The mouse corneal epithelial stem/progenitor cell line (TKE2) was divided into normal control group, high-glucose group and Slit2 treatment group.Western blot was performed to detect the expression of p-EGFR/EGFR and p-AKT/AKT in the TKE2 of the three groups.The expression of p-EGFR/EGFR and p-AKT/AKT in high glucose-cultured TKE2 with 0.01, 0.1 and 0.5 μg/ml Slit2 treatment for 10 minutes, and before and 10, 20, 30, 60, 120 minutes after 0.5 μg/ml Slit2 treatment was detected by Western blot.The effects of Slit2 on the axon regeneration of mouse trigeminal ganglion cells (TGs) were observed by immunofluorescence staining.The use and care of animals complied with the ARVO statement.This study protocol was approved by an Ethics Committee of Qingdao Eye Hospital of Shandong First Medical University (No.[2020]57).Results:At 48 and 72 hours after corneal epithelial scraping, the speed of corneal epithelial repair was significantly slowed down in diabetes model group in comparison with the normal control group and Slit2 injection group.The relative expression levels of Slit2 and its receptors Robo1, Robo2 and Robo4 mRNA in the normal corneal epithelium in the diabetes model group were significantly higher than those of the normal control group (all at P<0.05). The fluorescence intensity of Slit2 in normal corneal epithelium in diabetes model group was similar to the normal control group, and the fluorescence intensity of Slit2 in damaged corneal epithelium in diabetic mice was significantly weaker than that in normal control group.Corneal nerve plexus was denser at 7 days after corneal epithelial injury and the nerve fibers were increased with more branches in Slit2 injection group compared with diabetic group.The fluorescence intensity of p-EGFR, p-ERK, β-catenin and Ki67 in damaged corneal epithelium in normal control group and Slit2 injection group was stronger than that of the diabetes model group.The relative expression levels of p-EGFR/EGFR, p-AKT/AKT, and β-catenin in TKE2 in high-glucose group were significantly lower than those in normal control group and Slit2 treatment group (all at P<0.05). The relative expression levels of p-EGFR/EGFR and p-AKT/AKT in high glucose-cultured TKE2 after Slit2 treatment were significantly increased in comparison with before Slit2 treatment (both at P<0.05), and the relative expression levels of p-EGFR/EGFR and p-AKT/AKT in TKE2 were elevated as the increase of Slit2 concentration.The activation effect of 0.5 μg/ml Slit2 on EGFR and AKT pathways was most obvious.The synapse length of TGs cultured by high glucose was (40.52±5.44) μm, which was significantly shortened than (72.14±9.48) μm in normal control group and (73.04±4.66) μm in Slit2 injection group (both at P<0.05). Conclusions:Slit2 can protect the corneal epithelium by activating EGFR signaling pathway and play a protective role to neurons by increasing the density of corneal subepithelial plexus and promoting the growth of TGs axons in diabetic mice.

RESUMO Objetivo: Avaliar a possibilidade de alterações precoces sugestivas de ectasia detectáveis no Pentacam em casos sutis de alergia ocular em crianças. Métodos: Análise retrospectiva de 49 prontuários, com paciente de ambos os sexos e idades entre 3 e 14 anos. Do total de crianças estudadas, 31 apresentavam alergia ocular grau 1, e 18 não apresentavam qualquer sinal ou sintoma de alergia ocular, servindo como controles. Avaliaram-se diversos índices do Pentacam. Resultados: Houve associação estatisticamente significativa nas variáveis "Df - variação do mapa da elevação anterior da córnea" e "Pentacam combinado", que podem estar relacionadas com a detecção precoce de ectasia corneana nesses pacientes. Conclusão: Houve relação causal entre a prevalência da alergia ocular e alterações tomográficas na córnea, sugestivas de ceratocone em alguns pacientes provavelmente suscetíveis, podendo ser considerado um fator de risco para essa complicação.


ABSTRACT Objective: To assess if early alterations detected by Pentacam in children suffering from mild ocular allergy are suggestive of ectasia. Methods: A retrospective analysis of 49 medical records of patients of both sexes, aged between 3 and 14 years was performed. In this sample, 31 children suffered from grade I ocular allergy, and 18 presented no signs or symptoms of ocular allergy and comprised the control group. Several Pentacam indexes were evaluated. Results: A statistically significant association was found in the variables "Df - variation of the anterior corneal elevation map" and "combined Pentacam", which can be related to the early detection of corneal ectasia in these patients. Conclusion: There was a causal relation between prevalence of ocular allergy and tomographic alterations on the cornea, suggestive of keratoconus in some probably susceptible patients, which may be considered a risk factor for this complication.

ABSTRACT

@#Diabetic retinopathy caused by diabetes is one of the main causes of blindness and one of the most serious complications of diabetes. Recently, many scholars have foundthat in the early stage of diabetes before serious ocular complications, chronic injuries can still occur to anterior ocular tissues such as cornea, conjunctiva, meibomian glands <i>etc</i>. We review the impact of diabetes on the anterior segment of the patient with existing ophthalmologic examination techniques, which can help clinicians identify patients with early diabetes and then intervene and refer them, thus reducing the risk of long-term serious complications of diabetes.

The Notch signaling pathway plays an important role in cell fate and homeostasis.Various studies have proved that the Notch signaling pathway has strong effects on corneal wound healing and the maintenance of corneal homeostasis.Limbal stem cells inhibit differentiation and proliferation by inhibiting the Notch signaling pathway.Physiologic downregulation promotes cell migration and wound coverage in the early stage of corneal epithelial repair,and physiologic upregulation in the late stage of corneal epithelial repair is related to preventing excessive stratification of corneal epithelial cells and maintaining cell differentiation.Fibrosis is correlated with Notch after corneal stromal injury.The Notch signaling pathway is directly involved in the endothelial-to-mesenchymal transition induced by transforming growth factor-β after corneal endothelial injury.In addition,there are interactions between the Notch signaling pathway and 14-3-3 sigma,epidermal growth factor receptor,Sirt6,microRNA,and matrix metalloproteinases in maintaining corneal epithelial homeostasis,corneal epithelial differentiation,corneal stromal excessive inflammatory response,corneal neovascularization,etc.This review summarizes the function of the Notch signaling pathway in corneal wound healing.

@#Corneal disease is one of the main ophthalmopathy that may lead to blindness in China. Infectious keratitis is the main cause of corneal blindness. For corneal infections that are difficult to control by drugs, corneal transplantation is the only hope to control the infection and restore the sight. However, the demand for suitable donor corneas is increasing much faster than the number of donors, leaving thousands of curable patients untreated and even lose their eyeballs. Thus, it's urgent to seeking new biomaterials for corneal donor. In recent years, there have come forward great progress in bioengineering cornea. Here, we reviews its application present in clinic and prospects for the future, exploring the feasibility of bioengineering cornea being the replacement of human corneal donor in corneal transplantation.

ABSTRACT Purpose: To evaluate the complications and clinical results of Descemet membrane endothelial keratoplasty (DMEK) in patients with endothelial failure that occurred during the learning curve of a surgeon. Methods: Fifty eyes of 50 patients with DMEK and ≥6 months of follow-up were included. The patients were divided into the first 25 (group 1) and the second 25 (group 2) procedures performed by the surgeon. Best corrected visual acuity (BCVA), central corneal thickness (CCT), unfolding time of the Descemet membrane (DM) graft, and intraoperative and postoperative complications were compared between groups. Results: The differences in postoperative increase of BCVA (p=0.595) and decrease of CCT (p=0.725) in the two groups were not significant. The unfolding time of the DM was longer in group 1 than in group 2 (p=0.001). Primary graft failure occurred in three patients in group 1 and none in group 2. At the last visit, 42 (85.7%) of patients' corneas were clear, with significant difference between groups (p=0.584). A patient in group 1 with a history of pars plana vitrectomy, inferior iridectomy, and fluid as a tamponade experienced drop of the DM graft into the iridectomy space. All other intraoperative complications occurred in group 1. Conclusions: Occurrence of intraoperative and postoperative complications was increased in patients with coexisting ocular pathology or complicated endothelial dysfunction and during the surgeon's learning curve of DM endothelial keratoplasty procedures.


RESUMO Objetivo: Avaliar as complicações e os resultados clínicos da queratoplastia endotelial da membrana de Descemet (DMEK) em indivíduos com insuficiência endotelial, durante a curva de aprendizado de um cirurgião. Métodos: Cinquenta olhos de 50 pacientes submetidos ao procedimento queratoplastia endotelial da membrana de Descemet com pelo menos 6 meses de acompanhamento foram incluídos neste estudo. Os pacientes foram divididos em dois grupos: como os primeiros 25 casos do cirurgião (grupo 1) e como os 25 casos seguintes (grupo 2). A melhor acuidade visual corrigida (MAVC), a espessura corneana central (ECC), o tempo de desdobramento do enxerto da membrana de Descemet (MD), as complicações intraoperatórias e pós-operatórias foram apresentadas e comparadas entre os grupos. Resultados: Os grupos não diferiram estatisticamente em relação ao aumento pós-operatório de melhor acuidade visual corrigida (p=0,595) ou à diminuição da espessura corneana central (p=0,725). O tempo de desdobramento dos enxertos de membrana de Descemet no grupo 1 foi maior do que no grupo 2 (p=0,001). Falha do enxerto primário foi observada em 3 pacientes do grupo 1 e em nenhum do grupo 2. Na última visita, 42 (85,7%) das córneas dos pacientes estavam claras e não foram observadas diferenças estatisticamente significativas entre os grupos (p=0,584). Na cirurgia de um paciente do grupo 1, com história de vitrectomia pars plana (PPV) com iridectomia inferior e fluido como tamponamento, observou-se queda do enxerto de membrana de Descemet no local da iridectomia. Além disso, todas as demais complicações intraoperatórias ocorreram no grupo 1. Conclusões: As complicações intraoperatórias e pós-operatórias foram maiores em pacientes com coexistência de outra patologia ocular ou com disfunção endotelial complicada durante as curvas de aprendizado dos cirurgiões no procedimento queratoplastia endotelial da membrana de Descemet.

ABSTRACT

Keratoplasty is the main method for visual restoration once disease has affected corneal clarity.Because of its favorable surgical outcomes,deep anterior lamellar keratoplasty (DALK) has obtained popularity in ophthalmologist.It is of great importance to steadily spread the performance of DALK in China,which is otherwise prevented due to its high skill requirement and long learning curve.Therefore,this assessment reviewed the significance of spreading DALK in China,and discussed the indications,the learning curve and clinical research of DALK.

ABSTRACT

Optical coherence tomography (OCT),as a noncontact,innovative technology,can produce high-resolution images of ocular tissues.This technology is originally used for posterior segment in ophthalmology,which were widely utilized for the anterior segment at first.The development of anterior segment OCT (AS-OCT) technology enables the precise visualization of anterior segment structure.Moreover,ultrahigh-resolution OCT (UHR-OCT) has been helpful for diagnosis and management of corneal and anterior segment diseases,including quantification of tear film for dry eye,authentication of ocular surface squamous neoplasia (OSSN),pterygium,keratoconus,traumatic corneal injuries,etc.Besides,UHR-OCT can be also used for planning and performing surgery for anterior segment,such as corneal transplantation,and monitoring of epithelial wound healing and postoperative course as well.Moreover,this technology can help understand the physiology and pathophysiology of the anterior segment structure.Therefore,this review will give a review on the application of UHR-OCT for diagnosis and management of corneal and anterior segment disorders.

ABSTRACTPurpose:To evaluate the clinical outcomes of intrastromal corneal ring segment (ICRS) implantation to correct ectasia in eyes with prior refractive surgery.Methods:Forty-one eyes of 25 patients (13 men, 12 women; mean age, 28.66 years) with ectasia after refractive surgery [photorefractive keratectomy (PRK) or laser in situ keratomileusis (LASIK)] were included in a nonrandomized, retrospective, observational case series. Corneal tunnels were created by mechanical dissection in all eyes. Main outcome measures included uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BCVA), refraction, keratometry, and computerized analysis of corneal topography. Patients were divided into two groups by the type of refractive surgery (Group A: PRK, Group B: LASIK).Results:The mean preoperative manifest astigmatism decreased from -1.88 to -0.84 D in Group A (p=0.096) and -3.18 to -1.77 D in Group B (p=0.000). The mean keratometric astigmatism decreased from -2.58 to -1.66 D in Group A (p=0.010) and -4.80 to -2.78 D in Group B (p=0.000). The mean spherical equivalent decreased from -2.97 to -2.05 D in Group A (p=0.065) and -3.31 to -2.42 D in Group B (p=0.014). No significant between-group differences were noted on the comparison of preoperative and postoperative results. No intraoperative or postoperative complications were observed.Conclusion:ICRS implantation is a useful treatment option for ectasia following refractive surgery, and it has significantly reduced the refractive cylinder and increased best spectacle-corrected visual acuity.


RESUMOObjetivo:Avaliar os resultados do implante de anel intraestromal de córnea para correção de ectasia pós-cirurgia refrativa.Métodos:Quarenta e um olhos de 25 pacientes, 13 homens e 12 mulheres, com ectasia pós-cirurgia refrativa (PRK ou LASIK) foram incluídos em um estudo não randomizado, retrospectivo e observacional. A média de idade no momento do implante do anel é de 28,66 anos. Em todos os olhos, o túnel corneano foi criado através da dissecção mecânica da córnea. Os resultados avaliaram acuidade visual sem correção (AVSC) e acuidade visual com correção (AVCC), refração, ceratometria e topografia corneana computadorizada. Os pacientes foram divididos em dois grupos de acordo com a cirurgia refrativa. Grupo A: PRK, Grupo B: LASIK.Resultados:A média do astigmatismo pré-operatório foi reduzida de -1,88 D para -0,84 D no grupo A (P=0,096) e de -3,18 D para -1,77 D no grupo B (P=0,000). A média do astigmatismo ceratométrico foi reduzida de -2,58 D para -1,66 D no grupo A (P=0,010) e de -4,80 para -2,78 D no grupo B (P=0,000). A média do componente esférico foi reduzida de -2,97 D para -2,05 D no grupo A (P=0,065) e de -3,31 D para -2,42 D no grupo B (P=0,014). Nenhuma diferença estatisticamente significativa foi observada entre os grupos, quando se comparou os resultados do pré e pós-operatório. Não ocorreram complicações intra ou pós-operatórias.Conclusão:O implante de anel intraestromal de córnea é uma boa opção para o tratamento de ectasia pós-cirurgia refrativa, tendo resultado na redução significativa do astigmatismo refracional e melhora da acuidade visual com correção.

Objetivo: Avaliar o perfil epidemiológico dos pacientes submetidos à ceratoplastia no estado de Santa Catarina e as principais indicações para este procedimento. Métodos: Foi realizado estudo observacional, descritivo e retrospectivo com dados de 1161 pacientes transplantados entre janeiro de 2008 e dezembro de 2010, de acordo com os prontuários obtidos na Central de Notificação, Captação e Distribuição de Órgãos e Tecidos de Santa Catarina. As variáveis registradas foram: idade, sexo, procedência, data do transplante, indicação e olho operado. Resultados: A média de idade foi de 45,51 anos e o sexo masculino foi mais prevalente (54,05%). O ceratocone foi a doença mais frequente (36,09%), seguido por falência do enxerto (15,89%), leucoma (11,92%), ceratopatia bolhosa (11,06%), distrofias (7,77%), úlcera (5,36%), perfuração corneana (5,27%), descemetocele (4,66%), ceratite herpética (1,12%) e ectasia corneana (0,86%). A região do estado com maior número de casos foi a Grande Florianópolis (23,75%), sendo o ceratocone a principal indicação (30,91%). Leucoma e perfuração foram as principais indicações em pacientes com idade inferior a 10 anos, ceratocone nos pacientes entre 11 e 50 anos, falência do enxerto entre 51 e 60 anos e ceratopatia bolhosa nos pacientes acima de 61 anos. Conclusão: O ceratocone é a principal indicação para ceratoplastia no estado de Santa Catarina, com a média de idade de 31 anos. A maioria dos pacientes é do sexo masculino e proveniente da Grande Florianópolis. .


Purpose: To define the epidemiological aspects of the patients submitted to keratoplasty in the state of Santa Catarina, Brazil and the main indications for this procedure. Methods: We conducted an observational, descriptive and retrospective study with the data of 1161 patients submitted to cornea transplantation from january 2008 to december 2010, at the Transplantation Center of Santa Catarina. The analyzed data were: age, gender, origin, transplant date, indication and transplanted eye. Results: The mean age was 45.51 years and men were more commonly operated. Keratoconus was the most common disease (36.09%), followed by graft failure (15.89%), leukoma (11.92%), bullous keratopathy (11.06%), dystrophy (7.77%), ulcer (5.36%), corneal perforation (5.27%), descemetocel (4.66%), herpetic keratitis (1.12%) and corneal ectasia (0.86%). The region with the greater number of cases was the Great Florianopolis (23.75%), with keratoconus as the main indication (30.91%). Leukoma and perforation were the mainly indication in patients under 10 years old, keratoconus in patients between 11 and 50 years old, graft failure between 51 and 60 years old and bullous keratopathy in patients under 61 years old. Conclusion: Keratoconus is the main indication for keratoplasty in our state, with the mean age of 31 years. Most of the patients were men and from the Great Florianopolis. .

ABSTRACT

The amniotic membrane has been shown to have anti-inflammatory,anti-fibrotic,anti-angiogenic properties and ability to provide a substrate for the growth of corneal and conjunctival epithelial cells,and it is an ideal material for ocular surface reconstruction.The treating patten of corneal and ocular surface diseases has changed due to the widespread using of amniotic membrane transplantation,but there are a lot of problems in the application of amniotic memebrane in clinic,such as how to grasp indications and reduce the failure rate,how to apply individual skill for different patients with corneal disease,and how to observe postoperative complicationsm and reasonably use medicine after operation.This paper give some personal experience and opinion in orde to achieve better effects in treatment of corneal and ocular surface diseases using amniotic membrane transplantation.

Background In recent years,incidence of drug-induced keratopathy is increasing highly.Druginduced keratopathy is lack of typical clinical features and offen confused with the primary disease.Therefore,summarizing and concluding the clinicals feature and standard treatments of drug-induced keratopathy are key problem need to be solved urgently for us.Objective This study was to retrospectively analyze the clinical features and therapeutic procedure of drug-induced keratopathy.Methods A retrospective case series analysis method was adopted.The clinical data of 36 eyes (31 patients) with drug-induced keratopathy were collected by Shandong Eye Hospital from 2008 to 2012,including eye disease history,medication history,medication dosage and duration.A series of relevant examinations were performed,including best corrected visual acuity (BCVA) before and 1 month after treatment,Schirmer test Ⅰ (S Ⅰ t),tear film break-up time (BUT),meibomian gland findings,the location of the keratopathy,the characteristics of keratophthy before and after fluorescein staining.The treating were given,including cessating of the original drugs,applying corneal repair promotion and anti-inflammatory drugs as well as the comprehensive treatment for meibomian gland embolization and dry eye,such as the hot packs and massage in the eyes with meibomian gland dysfunction and a tear dot embolization therapy in the eyes with S Ⅰ t < 5 mm and BUT<5 s.Paired t test and repeated measured one-way analysis of variance in SPSS 17.0 software were used to compare the BCVA,BUT and S Ⅰ t outcomes.The correlation between corneal repair duration and S Ⅰ t results was analyzed by Pearson linear correlation analysis.Results The primary cause of drug-induced keratopathy was irrational use of drugs,including antiviral drugs,antibiotics,hormone,antiallergic,lowering-intraocular pressure drugs,turn for 23 eyes,12 eyes,10 eyes,1 eye and 1 eye,respectively.Improper route of administration included 25 cases of overuse of eye drops and 6 cases of subconjunctival injection.BCVA was 0.69 ± 0.28 1 month after treatment,which was significantly improved in comparison with before treatment (0.32 ± 0.26) (t =11.02,P < 0.01).Clinical manifestations included corneal epithelial diffusive and point-like roughness,corneal epithelial defect and even corneal ulcer for severe cases,corneal edema,Descemet membrane folds and partially visible corneal filiform.Drug-induced keratophthy was mainly located in the central and lower cornea.Comprehensive therapy was effective with the treating duration about 1 week to 8 weeks.A negative correlation was found between the corneal restore duration and S Ⅰ t results (r =-0.835,P<0.01).Conclusions Corneal changes secondary to topical medications may affect all layers of the cornea.Clinicians should be mindful of drug-induced ocular disorders.The early diagnosis of druginduced keratopahthy depends on the medical history and clinical features.A comprehensive treating based on ocular surface conditions is available.

ABSTRACT

PURPOSE: To report cases of transplanting a donor's 2 corneas to 5 patients suffering from several corneal diseases. CASE SUMMARY: Two corneas were donated from a 66-year-old donor, who suffered from brain damage due to asphyxia, one hour after being pronounced dead by doctors. Two penetrating keratoplasties and 3 partial lamellar keratoplasties were performed for patients with corneal opacity, corneal ulcer and corneal perforation. After the procedure all grafts were stable. CONCLUSIONS: Under the present circumstances of decreasing donations of corneas after death and the increasing demand for keratoplasty in Korea, the mutual cooperation among hospitals to treat more than one patient using one donated cornea is a method the authors believe can alleviate this situation.

ABSTRACT

Objective To evaluate the therapeutic efficacy of soft contact lenses on corneal diseases. Methods A total of 70 cases (82 eyes) of corneal diseases in the treatment group were treated with therapeutic soft contact lenses combined with topical antibiotic eye drops and artificial tears, but 64 cases (81 eyes) suffering from corneal diseases in the control group were treated with eye drops and eye ointments. ? 2 test was used to compare the cure rate of corneal epithelial defects and corneal ulcers caused by chemical and thermal burns between the two groups. Double sample t test was used to compare the average treatment time between the two groups. Results The cure rates of the corneal epithelial defects and corneal ulcers caused by chemical injury and thermal burn in the treatment group and the control group were 83% and 57%, respectively. In the treatment group, 100% patients with bullous keratopathy and 90% patients with dry eye disease were free from symptoms after wearing the lenses, but in the control group, the percentages were 50% and 63.6%, respectively. In the treatment group, 3 cases (4 eyes) of corneal epithelial defects caused by explosive injury and 3 cases of corneal epithelial defects after lameller keratoplasty were all cured, and 4 cases(4 eyes) of corneal perforations were healed. In the control group, 1 out of the 2 cases of corneal epithelial defects caused by explosive injury was cured, and 4 cases of corneal epithelial defects after lamellar keratoplasty surgery were cured, but 2 cases (2 eyes) of corneal perforation failed in the therapy, and corneal surgery was performed. In the treatment group, the cure rates of corneal epithelial defects or corneal ulcers caused by chemical injury and thermal burn were significantly higher than those in the control group ( P

ABSTRACT

PURPOSE: We collected genomic DNAs of Korean patients with inherited corneal disorders, and identified mutations of the BIGH3 gene related corneal dystrophies in Korean patients: Lattice type I (CDLI), Avellino (ACD), Reis-Buckler's (CDRB). METHODS: Slit-lamp examination of 75 patients and their relatives was carried out to confirm the diagnosis of the disorders. We composed pedigree and extracted genomic DNAs from members of the CDaffected family and individual patients. Genomic DNAs of the patients with mutation in BIGH3 gene were identified using Polymerase chain reaction (PCR) and sequencing. RESULTS: The number of patients was ranked as follows: 30 patients (40.0%) with ACD, 24 patients (32.0%) with keratoconus, 7 patients (9.3%) with CDLI, 5 patients (6.7%) with Fuchs' dystrophy, 3 patients (4.0%) with CDRB. In the genetic examination, we identified R124H mutation in ACD, R124C mutation in CDLI and R555Q mutation in CDRB. During this study, we also identified a polymorphism (F540F) in exon 12. CONCLUSIONS: ACD associated with R124H mutation is the most common form of inherited corneal disorder in Korea. And in Korean patients with CDRB, this study is the first report about mutation R555Q.

ABSTRACT

PURPOSE: To investigate the clinical manifestations and treatment results of Mooren's ulcer. METHODS: Sixteen eyes in eleven patients who were diagnosed as Mooren's ulcer at Seoul National University Hospital from 1995 to 2000 were enrolled. The clinical manifestations and treatment results were investigated retrospectively. RESULTS: The average age was 44.2 years and the male to female ratio was 5:6. The average follow up period was 26 months. Five patients had bilateral involvement. Nine patients were of limited type and two of progressive type. Ocular pain was the most frequently presenting symptom(82%). Topical corticosteroid application was the main therapeutic modality (75%) and all the patients responded well to the treatment. The prognosis was better in patients with unilateral lesion or in younger patients. As complications, one case of corneal perforation and one case of herpes keratitis occurred. CONCLUSIONS: Our retrospective study on Korean Mooren's ulcer cases revealed 1) there were few cases of progressive type, 2) most of them responded well to only topical steroid application, and 3) the prognosis was poor in patients with bilateral lesions or in old patients.
